🟡 Level 1 – Minor issue

Housekeeping or administrative irregularities with no direct threat to health or life: untidy workstation, missing markings, out-of-date documentation. Response: Resolution within standard timeframe.

🟠 Level 2 – Defect requiring correction

Irregularities that increase accident risk: worn personal protective equipment, partially obstructed evacuation routes, absence of current Health and Safety instructions. Response: Urgent resolution with monitoring of corrective actions.

🔴 Level 3 – Serious hazard

Defects that could lead to injury or a serious incident: damaged machine guards, non-functioning fire-fighting equipment, work carried out without required authorisations or training. Response: Immediate corrective action, possible restriction of work.

🚨 Level 4 – Critical hazard

Direct threat to life or health: work at height without fall protection, live exposed electrical installations, hazardous substance leak. Response: Immediate work stoppage until the hazard is eliminated.
